Our Mission:
Liberation Programs' mission is to provide prevention, treatment, harm reduction and recovery services to help individuals and their families impacted by substance use and mental health conditions to foster hope and maintain wellness.
Who We Are:
Liberation Programs is one of Fairfield County's leading behavioral health organizations specializing in treatment for all types of substance use disorders including alcohol, opiates, depressants and stimulants. Our goal is to help individuals and families overcome addiction in order to restore their lives and ultimately strengthen our communities. Liberation provides services for youth, adults and families that include two inpatient treatment programs, outpatient and intensive outpatient services, health education for older adults and people living with HIV/AIDS and other chronic illnesses, treatment and resources for adolescents and their families, education and prevention efforts in the community, and permanent supportive housing for families. In operation since 1971, the agency has sites in Greenwich, Stamford, Norwalk and Bridgeport and served more than 2,400 individuals in our last fiscal year.
Greenwich Connects is a community-based initiative designed to support young adults (ages 18–25) and their families in Greenwich. This program provides mental health and wellness coaching, family engagement opportunities, and connections to essential services. The Family Life Coach is a trusted navigator and support system, supporting young adults to build resilience, access resources, and strengthen family relationships in a non-clinical, welcoming environment.
Key Responsibilities:
Mental Health and Wellness Support
1. Navigate & lead one-on-one and group meetings with young adults to provide coaching, supportive listening, and resource navigation.
2. Offer mental health support through informal assessments, regular follow-up, and referrals—without requiring a formal diagnosis.
3. Promote mental wellness in a stigma-free, conversational environment that encourages trust and openness.
Outreach and Engagement
1. Conduct targeted outreach throughout the community to engage disconnected young adults through local schools, colleges, faith-based groups, and other community partners.
2. Build visibility for the Family Life Coach program through presence at the YMCA and other community settings.
Family-Centered Engagement
1. Engage families in coaching sessions and educational activities aimed at fostering stronger relationships and reducing isolation.
2. Coordinate and facilitate educational workshops and Fathers Forum on topics related to family dynamics, mindfulness, and youth development in partnership with Greenwich community partners.
3. Serve as a consistent point of contact and support for parents and/or caregivers navigating concerns related to young adult mental health and life transitions.
Resource Coordination and Referrals
1. Connect participants to resources by request including but not limited to; education, employment, housing, food assistance, and as relevant Liberation’s clinical services (e.g., Greenwich Youth Options).
2. Maintain accurate records of encounters, engagements, and follow-up using Liberation’s electronic health record system (NetSmart EHR).
3. Track participant satisfaction and program outcomes through agency surveys and data collection tools.
Qualifications:
1. Bachelor’s degree in social work, human services, psychology, or a related field (Master’s Degree preferred).
2. Demonstrated experience working with youth, young adults, or families in a supportive services setting.
3. Demonstrated ability to engage, build rapport quickly and work compassionately with diverse populations.
4. Strong understanding of mental health challenges facing young adults, especially those who are disconnected from school, work or the community.
5. Knowledge of resources in the community and ability to facilitate referrals.
6. Excellent organizational and communication skills.
7. Proficiency with Microsoft Office and electronic health records (or willingness to learn).
8. Ability to work a flexible schedule including, mornings, evenings and weekends to accommodate program needs including coordination and facilitation of events.
