JOB SUMMARY:

Works under the supervision of the Branch Nursing Leader, supervises, orients, instructs and monitors professional staff rending care to patients in their homes. Receives and assists with assignment of nurses for new patients. Directly involved in hiring of new field staff, provides orientation and educational training. Serves as a liaison between the community and the agency. Assists the Branch with quality management program and performance improvement projects such as chart audits, formulation of Plan of Care (485) and assessing staff’s clinical documentation techniques, etc.

MAJOR DUTIES AND RESPONSIBILITIES:

1. Demonstrates ability to accept and handle leadership role. Communicates effectively with a variety of different people, both verbally and in writing. Firm grasp of basic nursing concepts and principles of delivery of quality patient care.

2. Understands the overall operations and functions of a home health program.

3. Assists Director in creating an environment in which home health staff can work together, in which each has an opportunity for self-expression, for creating a sense of accomplishment and for personal growth.

4. Maintains adequate knowledge in documentation requirements to serve as resource for staff.

5. In collaboration with Director interviews and assists with selection of qualified personnel. Provides input for staff evaluations. Performs disciplinary action as required after consultation with Branch/HR.

6. Assist with field orientation of newly hired home infusion nurses as necessary.

7. Performs direct patient care activities to patients in their home that are in compliance with all requirements of company policies and processes for Registered Nurses.

8. Oversees day to day delivery of skilled nursing visits for patients in assigned territory.

9. Assists Director in conducting staff meetings, in-services, and other meetings as assigned.

10. Works cooperatively with all professional health members, interfaces with Branch to participate in team conference and provides for continuity of care and coordination with other departments.

11. Assures accurate, current medical records are maintained according to state and federal requirements, maintains patient confidentiality.

12. Acts as liaison between the physician, nurse, pharmacist and patient/family when problems arise.

13. Assist Director in obtaining documentation for personnel files according to policy and laws.

14. Seeks assistance from appropriate resource person as necessary to solve clinical and management problems, utilizes skills to motivate staff and resolve conflicts.

15. Demonstrates ability to be flexible in changing work environment, works well in problem solving situation.

16. Follows communication needs of changes to staff in agency policies and procedures.

17. Assists with patient scheduling needs.

18. Other tasks as assigned.

SKILLS AND ABILITIES:

• Essential Technical/Motor Skills: Office sitting, use of car, copy machine, computer paper cutter, fax machine, Infusion pumps, BP cuff/stethoscope, telephone.

• Interpersonal Skills: Able to listen interact with staff, respond verbally, able to hear clearly to communicate on telephone, public speaking, write legibly-clearly.

• Essential Physical Requirements: Sitting for long periods of time, standing, walking, crouching, stooping, reaching, twisting, climbing stairs, kneeling, handling/grasping, balance, driving.

• Essential Mental Abilities: Able to make favorable impression always, maintain positive attitude, adapt priorities, work must be self-matured independently meeting deadlines.

• Essential Sensor Requirements: Vision 20/40 to read, write, type and operate an automobile, have a good sense of smell to identify hazards, clear hearing to listen to staff, touch to feel temperature.

• Exposure to Hazards: Dust, air conditioning, heat fumes.

• Others: This job may require the employee to be responsible for care of patients of all ages, must be competent in ages she/he supervises.

EDUCATION AND/OR EXPERIENCE:

· Nursing Degree from a program accredited by the National League for Nursing, BSN preferred

· Current RN license in all states of practice, driver’s license, auto insurance, CPR card.

· At least two years’ experience as a registered nurse in a home health agency or its equivalent in the past five years, two years supervisory experience preferred.

· Knowledge and Abilities:

a) Demonstrates knowledge and skills necessary to provide care to and communicate with patients.

b) Able to assess data reflecting the client’s status and interpret the appropriate information needed to identify each client’s requirement relative to their age-specific needs.
