\tSchedule: Varies based on unit needs
\tSchedule issued in 4-week blocks
\tWeekend Coverage: Rotational (required)
\tCall Required: Yes
\tFloating Required: Yes
\tStaffing Ratio: 1 Case Manager : up to 25 patients
\tMinimum 2 years of Acute Care Case Management experience REQUIRED
\tMust have experience performing Care Coordination and Discharge Planning
\tTravel experience REQUIRED (first-time travelers NOT accepted)
\tEpic experience REQUIRED (HARD STOP)
\tCase Managers DO NOT perform Utilization Review
\tThis role is focused on:
\tCare coordination
\t\tDischarge planning
\t\tPatient and family support
\t\tBLS Certification
\tAssess patients and develop individualized discharge plans
\tPerform care coordination across the continuum
\tOrganize and participate in family meetings
\tCollaborate in development of treatment plans
\tArrange post-acute services independently, including:
\tDME
\t\tSNF placement
\t\tAmbulance transportation
\t\tCoordinate with:
\tPhysicians
\t\tNursing staff
\t\tSocial services
\t\tExternal providers and payors
\t\tMaintain accurate, timely documentation in Epic
\tEnsure compliance with HIPAA and regulatory standards
\tAcute care hospital case management workflows
\tExperience with HMOs, IPAs, and managed care plans
\tDischarge planning for complex social situations, including:
\tHomelessness
\t\tMental health history or crisis
\t\tSubstance abuse treatment options
\t\tAdoption / foster care processes
\t\tAdvance directives & POLST
\t\tProtective services processes
\t\tStrong communication and organizational skills
\tAbility to manage a high-volume caseload independently
\tCommunity hospital experience
\tFamiliarity with regulatory standards (CMS, Joint Commission, HIPAA)
\tNursing
